What do you need to qualify for WIC?

My boyfriend wants me to apply for WIC. I don't know how to apply for it. What do I need to take with me when I do go and apply? I live with my mom and dad, do not pay any utilities and my car is under my name and my boyfriend's name and the cell phones are under my boyfriends name. However I did qualify for Medicaid so since I did qualify for Medicaid does this mean I should qualify for WIC?
